Order cancelled without a valid reason - I placed 2 separate orders for the same book for 2 separate students we are sponsoring through class. 2nd order cancelled due to "similarity" to previous purchase? I guess?
So, this order was actually a re-order.
Processed, not cancelled, and delivered within the stated time frame.

When you require people to type out responses instead of just selecting stars it makes them really not want to do the survey just a heads up. I don't have time to sit and type out things but typically I will do the star or select a rating process. For instance if there's another part to the survey I'm certainly not going to do it because you're asking me to fill out a whole bunch of stuff
