Refused to fill out landlord form and it's asking the same information that on the ESA form I'm confused. Say they are not comfortable filling it out. I already put in my notice to move and now I can't bring my dog to my new hour because they refuse to fill out form

I had no difficulties purchasing a package but afterwards when I have to submit vest size and actual address there is a page error. I've tried to refresh and re-do this quite a few times. So now they have my money but no actual address to send the product to. Called the help line only to wait 30 min for an automated voice saying someone will call me back. Is this a scam? Send me the products I paid money for please.
