I needed the form within 48 hours (like is promised) because I was moving into a new apartment that didn't allow pets. It has been over 2 weeks and the form has never come. I have emailed them multiple times, and every time they just say that they will review my order, but nothing has changed. There is no representative that you can talk to over the phone, as their phone message is just a robot.

This is a renewal for Kobe, we needed assistance thru last year from the doctors and they prevailed, I would recommend them.
I was scammed and asked for a refund and they want a review instead. Did not receive anything 5 days now they are referring me to another agent over and over. They want more money.
