Requested a copy of my daughter's birth certificate. Sent the notorized form and application as requested. Received email everything received, checked and 100% accepted and would receive the birth certificate in 35 to 40 days. 60 days later nothing received.

The site popped up when I was looking at the Massachusetts main dot gov page. I bought into it, at cost of $94. For that, once you have provided your information (parents' names, ones place and date of birth, etc.) you are sent, for downloading and printing, the Massachusetts standard application form with your data typed in. This form is free if just downloaded and printed from the dot gov page. You just write your own info in.

I applied and received my application to mail to Indiana State Department of Health and mailed it with my check. You did not specify what the "Order" was, but I assumed it was the application, so I have responded that I received that. However, there is nothing that gives me even a possible date that I might receive my Certified Birth Certificate.

The service provided is way over priced. I did not find the website without problems or difficulty. It kicked out my payment three times with two different cards and a fraud alert. I should have not been so persistent to make a payment. Finding you only provide a application is a ripoff. I should have avoided your site and went to the vital records for PA. It would have only cost me.$20 and $10 for on line. Your website cost me $59 and had to turn around and pay the $20 to the State for the birthday certificate.
