Too much info that you have to put in. Why do you need to know our insurance policy number? Seems excessive. You should also really consider giving the WHOLE price of what your buying instead of tricking people into thinking it would be one price when you charge them $40 extra without knowledge. I recommend gorenew.com over this site anyday.
