You ask a bunch of questions and then end by asking for personal info that you can sell. You say "you can trust us", but if you really want to prove that then don't require a phone number. Wait for the user to be comfortable providing that info. Seriously, I'm sick of spam calls, just like everyone else. Every time I add my number to one of these forms, I start getting more spam.
